Job Summary:

We are seeking a compassionate, motivated, and multilingual Foreign Medical Graduate (FMG) to join our Chronic Care Management (CCM) team in a fast-paced, patient-centered primary care practice. This is a unique opportunity for an FMG to gain meaningful, hands-on clinical experience under the supervision of licensed physicians while contributing to direct patient care, education, and care coordination.

Key Responsibilities:
  • Engage in direct patient interaction including patient outreach, care coordination, and education under physician supervision.

  • Conduct patient interviews, gather and document medical histories, and assist in chronic care management workflows.

  • Support and help implement care plans for patients with chronic conditions.

  • Provide patient education on treatment plans, preventive care, and disease management.

  • Assist in collecting and reporting healthcare metrics, patient outcomes , and compliance data aligned with value-based care models and report cards.

  • Use EMR systems to maintain accurate and updated patient documentation.

  • Maintain strict confidentiality and adhere to HIPAA regulations and internal compliance standards.

  • Participate in team meetings and quality improvement initiatives.

Qualifications:
  • Medical degree from an accredited foreign medical school .

  • Strong clinical knowledge and familiarity with U.S. primary care principles .

  • Bilingual or trilingual required - must be fluent in English and Spanish and/or Portuguese .

  • Excellent interpersonal and communication skills with a strong commitment to patient care.

  • Ability to function in a team-based care model .

  • Familiarity with healthcare compliance, report cards, and patient statistics is preferred.

  • Proficient with (or ability to learn) electronic medical record (EMR) systems.

  • ECFMG certification preferred , but not required.

  • Must be authorized to work in the United States .
